Many insurance companies require a call from the patient in order to open a case and/or begin pre-authorization for services prior to your initial appointment.
Please verify with your insurance company that the doctor or clinician with whom you are scheduled is an in-network provider.
Effective March, 2006 initial for medication evaluation will require a $50 cash or credit card deposit to schedule the appointment.
